Obverse description Uniformed bust of King Rama X (Vajiralongkorn) facing left, rendered in high relief against a plain field. The royal full ceremonial title in Thai script curves along the left periphery, with additional inscriptions arcing along the right periphery. The portrait is uncrowned and depicts the king in military dress uniform, with fine detail on the collar and lapels.

Reverse description Central device depicts the emblem of the Faculty of Medicine, Vajira Hospital, Navamindradhiraj University, featuring a deity mounted on a front-facing elephant. The motto of the university in Pali script, 'Vijjā Uppattaṃ Seṭṭha' (Of all things that have come to pass, knowledge is the most excellent), appears above the central device. The university name in Thai script is inscribed below the central device, all enclosed within a double circle. Commemorative legends surrounding the design note the 110th anniversary of Vajira Hospital, the date 2nd January BE 2565, the denomination '20 บาท', and 'ประเทศไทย' (Thailand).
