Obverse description The obverse features a dual-plane composition in which the fourth-generation effigy of Queen Elizabeth II by Ian Rank-Broadley, facing right, is set within a recessed circular medallion occupying the upper centre of the field. Surrounding the portrait, the broader field presents an elaborately textured Romanesque decorative ground incorporating two ornate processional crosses in high relief — one to the left and one to the right — alongside interlacing scroll and bead motifs evoking early medieval Bohemian goldsmith art. The legend arcs along the upper rim reading ELIZABETH II · NIUE ISLAND · 100 DOLLARS · 2021, while additional inscriptions in the field record the engravers' initials FZ and IRB, the fineness AU 999,9, the weight 2 oz, and the mint mark CM.

Reverse description The reverse presents facing bust portraits of Saint Ludmila and Saint Wenceslas rendered in a refined medieval icon-inspired style. Saint Ludmila appears on the left, depicted facing slightly right, wearing a jewelled crown and veil with a radiant nimbus; Saint Wenceslas appears on the right, facing slightly left, adorned with an ornate bejewelled crown and richly embroidered ceremonial vestments, also with a nimbus. Between the two figures, in the upper centre, the Bohemian heraldic eagle is shown on a shield.
