| Obverse description | Standing figure of St. John the Baptist at left, nimbed and robed, facing right, holding a long patriarchal cross staff in his right hand; to his right, the Grand Master Gregorio Carafa kneels in supplication, wearing armour and a ducal mantle, his hands raised in prayer. The two figures flank the central staff in a devotional composition closely derived from the Venetian zecchino tradition. A circular Latin legend surrounds the scene, reading F·D·GREG·CARAFA S·IO·BAPTISTA, distributed around the inner border of the coin. |

| Reverse description | Crowned quartered coat of arms of the Order of Malta and the Carafa family displayed prominently in the centre of the field, the shield surmounted by a princely crown and flanked by decorative mantling. The upper quarters bear the white cross of the Order on a black field alternating with the Carafa family arms. The entire achievement is enclosed within a beaded inner border. The surrounding Latin legend DE PRINC·ROCELL M·M·HIER·ET·S·S·H· refers to Carafa's titles as Prince of Roccella and Grand Master of Jerusalem. |
