Obverse description	Printed in dark reddish-brown on a light ground, the obverse carries the heading CHEQUE PROVISIONAL DE CIRCULACION FORZOSA at the top, beneath which a curved panel bears the guarantee inscription EMITIDO CON LA GARANTIA DE LA DEUDA DEL ESTADO, RENTAS FISCALES Y DEPARTAMENTALES. To the left a winged allegorical figure stands amid a trophy of arms and laurel branches, while the central field presents a large numeral 1 flanked by guilloche ovals, with the place and date IQUITOS, 1º OCTUBRE 1921 and the denomination UNA LIBRA PERUANA DE ORO below; two manuscript signature lines for the CAP. JEFE POLITICO Y MILITAR and the ADMOR. DE LA ADUANA appear at the foot alongside the serial number.

Reverse description	Printed in red on a plain ground, the reverse centres on the Peruvian national coat of arms within a circular vignette enclosed by ornate guilloche scrollwork, with CHEQUE PROVISIONAL in two lines at the top. The numeral 1 is repeated in each corner within decorative frames of interlocking wheel-and-floral ornaments, and the denomination UNA LIBRA PERUANA DE ORO occupies a rectangular panel at the foot; a circular handstamp cancellation is visible at the left.
