| Obverse description | The obverse features a uniformed bust of Yuri Lysiansky in three-quarter view facing right, with a tall-masted sailing ship depicted in the right field. To the left, inscriptions on a scroll-like panel identify him as a descendant of a Cossack officer family from Nizhyn, with his life dates 1773–1837. The border is formed by a decorative chain motif encircling the entire field, with the legend identifying him as a Captain of the 1st Rank arranged along the lower rim. |

| Reverse description | The reverse displays the elaborate heraldic coat of arms of the Lysiansky family in high relief at the center, featuring supporters, a crest with an animal figure, and a shield, set on a decorative base. A circular legend around the upper field reads the dates of the round-the-world voyage 1803–1806, with the names of Kruzenshtern and Lysiansky arranged around the lower border. The year 2021 appears to the left of the coat of arms, and the entire design is framed by the same decorative chain border as the obverse. |
