Obverse description Two full-length figures in relief occupy the central field: Captain Cook, depicted in 18th-century dress and hat, extends a medal toward a Tasmanian Aboriginal figure facing him. The legend NUMISMATIC ASSOCIATION OF AUSTRALIA INC arcs along the upper rim, with the inscription CAPTAIN COOK PRESENTS MEDAL to the lower left and TO TASMANIAN ABORIGINAL IN 1777 to the lower right. The Adelaide Mint monogram AM and the NAA logo appear in the lower exergue.

Reverse description A detailed relief depiction of the clipper ship City of Adelaide under full sail occupies the central field, shown underway on the sea. The legend NAAC2015 arcs along the upper rim, with ADELAIDE in large letters along the lower rim. The inscription BOUND FOR SOUTH AUSTRALIA curves across the lower field, and City of Adelaide 1864 appears in the central lower area below the vessel. The Adelaide Mint monogram AM is visible to the lower left of the ship.
