Obverse description Central field displays the arms of Liverpool: a shield bearing the Liver Bird, supported on the left by Neptune (crowned, semi-draped, holding a trident) and on the right by a mermaid, both flanking two flagpoles. Above the shield, a crest featuring a Liver Bird with wings extended stands atop a cormorant with wings folded. Below the shield, a ribbon scroll bears the motto legend. The whole is enclosed within a beaded border with the committee legend around the periphery.

Reverse description The reverse features a multi-line award inscription in raised lettering within an open field, flanked on both sides by relief laurel branches tied with a ribbon bow at the base. The recipient's name is engraved in cursive script above the printed text, and the year of award appears within a raised rectangular cartouche at the foot of the inscription.
