Obverse description A nude male figure in a crouching diving stance is depicted in high relief within a rectangular recessed field, poised to dive from a starting block. Flanking the central field on either side are sprays of oak and laurel branches in relief. The top of the medal features decorative foliage ornament below a suspension ring.

Reverse description A central circular cartouche bears the legend in three lines, with the club name arching along the upper curve. Crossed oak and laurel branches frame the lower portion of the cartouche. A blank beaded rectangular panel occupies the lower field, intended for engraving. Scrollwork ornament decorates the upper portion of the reverse.
