Obverse description High-relief bust of Marie-Thérèse Haze facing slightly left, depicted in the habit of a religious sister with a large wimple and a cross pendant at her chest. The name MARIE THÉRÈSE appears in the left field and HAZE in the right field, both in raised relief following the curve of the medal. A small engraver's signature appears in the lower field.

Reverse description High-relief depiction of the Collegiate Church of Saint Bartholomew in Liège occupying the lower portion of the field, with the emblem of the Daughters of the Cross — a cross with a circular element — rising prominently to the right. The legend CONGREGATION DES FILLES DE LA CROIX arcs across the upper field, and the date 21·IV·1991 is inscribed in the lower exergue. The engraver's signature MABA appears to the right.
