Obverse description The obverse features four portrait busts in high relief depicting George Washington, Benjamin Franklin, Thomas Jefferson, and John F. Kennedy arranged across the lower field, with a bald eagle displayed in the upper center. Surrounding historical vignettes include Independence Hall, stripes and stars of the American flag, the Spirit of '76 figural group, and a Saturn V rocket at launch, all framed by the peripheral legend and a lower inscription.

Reverse description The reverse is divided into thirteen radiating wedge-shaped segments, each containing a raised outline map of one of the original thirteen states accompanied by the state's name and ratification date in incuse lettering. A five-pointed star in sunken relief occupies the central roundel, and the overall composition mirrors the tridecagonal shape of the medal.
