Obverse description Laureate, draped, and cuirassed bust of Emperor Macrinus facing right, portrayed three-quarter view from the rear, with the paludamentum visible over the left shoulder and the segmented armour of the cuirass rendered in detail. The effigy is enclosed within a beaded border, with the Greek imperial titulature legend distributed around the periphery of the flan. The portrait displays the characteristic bearded physiognomy of Macrinus as known from other provincial issues of this reign.

Reverse description Heracles depicted nude, standing to the right, engaged in the First Labour as he wrestles and strangles the Nemean lion, which is shown rearing up before him; the musculature of the hero is rendered with provincial vigour. The composition is set within a beaded border, with the ethnic legend of the issuing city placed in the field. The reverse type reflects the civic pride of Cius in its mythological associations with the Heraclean tradition of Bithynia.
