| Obverse description | Facing bust portrait of Mother Teresa, depicted wearing her characteristic white habit and headscarf with blue trim, rendered in high relief against a frosted field. The portrait occupies the central device and captures her aged visage in realistic detail. The legend MOTHER TERESA arcs across the upper border of the black-enamelled rim, flanked by the birth and death years 1910 to the left and 1997 to the right. The motto SERVE LOVE GIVE appears in three separate words along the lower arc of the enamelled border. |

A ribbon scroll beneath the wreath bears the legend KNIGHTS OF MALTA. Above the crown, a small Maltese cross finial is visible. The denomination 500 LIRAS appears in large characters along the lower exergue, flanked by the split date 20-03 and the initials O.S.J. below the central device.
