Blank planchets escaping the mint before striking are errors of opportunity — they slip through when press operators or quality control staff fail to catch unstruck discs moving along the feed system. Indonesia's state mint in Semaran (later consolidated under Perum Peruri, established in 1971) was scaling up domestic production capacity precisely during this period, making process control lapses more plausible than in a long-stabilized operation.
