Heilstätte Steinklamm was a sanatorium, and this 50 Heller note is Lagergeld — internal scrip issued for use within the institution rather than a public currency. Austrian sanatoria, particularly those operating through the late imperial and early First Republic periods, routinely issued their own small-denomination paper to manage canteen and amenity transactions among patients, insulating the facility's internal economy from outside circulation.
J. Radinger of Mariazell was a local job printer, not a security printing house. The absence of any serious anti-counterfeiting measures was considered acceptable given the captive, controlled user base.
