Obverse description The Polish state eagle displayed with wings spread, wearing a royal crown, occupies the upper portion of the field. The numeral '5' appears in large relief to either side of the eagle's talons, with the denomination legend 'ZŁOTYCH' inscribed horizontally below, and the date '1930' in the exergue beneath the eagle's body. The design is contained within a fine inner border of beading.

Reverse description A military standard bearing a banner inscribed 'HONOR I OJCZYZNA' (Honour and Fatherland) dominates the upper field, referencing the insignia of the November Uprising of 1830. The word 'PRÓBA' (trial/pattern) is inscribed to the upper left of the field. Below the standard, the commemorative legend is arranged in four lines across the lower field, flanked by the dates '1830' and '1930' denoting the centenary of the uprising. A mint mark appears in the lower exergue. The entire design is enclosed within a beaded inner border.
