| Obverse description | Centered in the field is the Polish white eagle displayed, crowned, with wings spread and talons clasping orb and sceptre, rendered in high relief in the Art Deco style. The circular legend RZECZPOSPOLITA POLSKA arcs along the upper periphery in bold Latin capitals. The date 1925 is inscribed at the bottom of the field, flanked by two small decorative stars. The designers' monograms SW (Stanisław Szukalski) and WG (Wojciech Jastrzębowski) appear in the lower field to either side of the eagle's talons, with an additional engraver's mark visible at lower left. |

| Reverse description | The reverse presents an allegorical figural composition in the Art Deco style designed by Edward Wittig. A seated female personification of Polonia, draped and crowned, extends her right arm toward a standing young soldier or youth who holds a shield bearing the word KONSTYTUCJA (Constitution). At the base of the composition lie sheaves of wheat and other symbolic attributes. An inner beaded border of 100 beads encircles the entire design. The denomination legend 5 PIĘĆ ZŁOTYCH 5 is inscribed along the lower exergue, with the numeral 5 flanking the written value on either side. |
