Obverse description A front-facing bust portrait vignette of Emilio Aguinaldo occupies the left centre of the note, set against a multicolour guilloche underprint. The Bangko Sentral ng Pilipinas seal is positioned at right centre, accompanied by a vignette of the historical marker at Barasoain Church and a cannon rendered in intaglio at lower right.

Reverse description The central vignette presents an intaglio scene commemorating the Declaration of Philippine Independence on 12 June 1898, with Emilio Aguinaldo and assembled dignitaries on a decorated balcony above a jubilant crowd of armed revolutionaries raising rifles, flags, and hats in celebration. Denomination numerals "5" appear in large intaglio figures at both left and right margins, with a multicolour guilloche underprint in orange, green, and red tones framing the composition. The legend "LIMANG PISO" is inscribed in bold letterpress along the lower border.
