Obverse description Central field features the municipal coat of arms of Polzin, a divided shield displaying horizontal stripes on the left half and a stylized bunch of grapes or hops on the right half, rendered in low relief. The legend STADT POLZIN arcs along the upper periphery in raised Latin capitals. The date 1919 appears in the lower field beneath the shield. Two six-pointed stars flank the shield at the lower left and right, and the design is enclosed within a beaded border.

Reverse description The reverse is dominated by the large numeral 5 in the central field, denoting the face value. The word NOTGELD arcs along the upper periphery in bold raised Latin capitals, and the denomination legend PFENNIG is inscribed in a straight line across the lower field. Two six-pointed stars flank the numeral at mid-field. A small decorative lozenge-shaped ornament appears at the very bottom of the field, and the entire design is enclosed within a beaded border.
