Obverse description Black intaglio print on white paper with red guilloche underprint. At left, an oval vignette of a female portrait in classical style, encircled by the inscription REPUBLICA DE GUATEMALA and the word CINCO. To the center-right, a larger vignette of a seated allegorical female figure. The date 2 de Noviembre de 1921 and place QUEZALTENANGO are printed above the serial numbers, with the denomination CINCO PESOS in large letterpress at the bottom center.

Reverse description Printed entirely in green intaglio on white paper with dense guilloche work filling the field. The Guatemalan national coat of arms occupies the central medallion, flanked by the large numeral 5 on both left and right. Ornate lathe-work borders frame the entire design, with the bank name and place of issue inscribed at top and bottom respectively, and the printer's imprint of Waterlow & Sons Limited, London at the foot.
