At upper centre, a circular vignette bears the bank's seal inscribed BANCO DE VALPARAISO / PISAGUA enclosing a Chilean coat of arms, flanked by decorative lathe-work panels. The upper margin carries the bank's name in large Gothic script, with the note number field at upper left, the denomination $5 at upper right, and the branch designation Oficina de Pisagua with a manuscript date at mid-left; the body text, in cursive script, states the deposit obligation for CINCO PESOS moneda corriente payable in three months without interest, with signature lines for Cajero and Agente at the foot.
