Obverse description The obverse is divided into four quadrants by lines radiating from a central ornamental rosette, at the hub of which appears the international radioactive hazard symbol. The upper-left quadrant depicts a black stork standing in profile; the upper-right shows a bear; the lower-left portrays a Przewalski's horse; and the lower-right features a moose and a bison, all rendered in high relief against a plain field. At the upper centre, the Ukrainian trident coat of arms (tryzub) is displayed to the left of the large numeral '5', with the legends 'УКРАЇНА' and 'ГРИВЕНЬ' flanking the central rosette. The bilingual circular legend 'ЧОРНОБИЛЬСЬКИЙ РАДІАЦІЙНО-ЕКОЛОГІЧНИЙ БІОСФЕРНИЙ ЗАПОВІДНИК / CHORNOBYL RADIATION AND ECOLOGICAL BIOSPHERE RESERVE' runs along the outer border, with the date '2024' at the bottom.

Reverse description The reverse features a full-body portrait of an adult Black Stork (Ciconia nigra) rendered in vivid applied colour, standing erect at centre-left amid a naturalistic wetland setting with green reeds and grasses depicted in colour. The bird's characteristic iridescent black plumage, white underparts, and bright red bill and legs are faithfully reproduced. To the right, an uncoloured relief vignette depicts a nest in a bare tree with chicks, set against a tranquil riverscape with a treeline in the background. The species name legend 'ЛЕЛЕКА ЧОРНИЙ' in Cyrillic and its binomial nomenclature 'CICONIA NIGRA' in Latin appear in the upper-left field.
