| Obverse description | Printed in green on cream paper, the obverse carries a tall arched vignette at left enclosing a standing allegorical female figure in classical robes set against a landscape background, with a circular ink stamp overlapping the lower left corner. At upper centre the inscription VILLE D'AMIENS appears above BANQUE DUVETTE, with the city arms in a cartouche at upper right. The denomination CINQ FRANCS is set in large bold letterpress across the centre, below which the date 15 Septembre 1914 is printed, followed by the manuscript signatures of Le Maire, L'Emetteur, and Les Délégués de la Chambre de Commerce, with a boxed serial number at lower centre and the printer's imprint IMP. DU PROGRÈS at the base. |

| Reverse description | The reverse is printed entirely in green and displays a dense ornamental guilloche composition covering the full field, formed by stylised bamboo stalks and foliage arranged in repeating vertical panels separated by foliate borders. A central rectangular frame encloses the large numeral 5 surrounded by a fine floral guilloche ring, while the digit 5 is repeated in small boxed counters at each of the four corners. The outer border consists of a continuous geometric lace-pattern frame. |
