Obverse description Right-facing draped bust of Her Majesty Queen Elizabeth II, wearing a tiara, rendered in high relief against a mirror-proof field. The effigy, based on the Ian Rank-Broadley portrait, is centrally positioned and finely detailed. The circumferential legend reads ELIZABETH II to the upper left and COOK ISLANDS to the upper right, with the date 2009 to the right and the denomination 5 DOLLARS along the lower arc.

Reverse description Central effigy of Neptune, the Roman god of the sea, depicted as a muscular bearded figure seated on a rocky outcrop and holding a trident in his raised left hand, rendered in high relief against a color-printed background showing the planet Neptune in shades of blue. The astronomical symbol for Neptune appears to the right of the central figure. Two decorative curved lines flank the lower field, and five-pointed stars punctuate the border.
