| Obverse description | Rectangular flan with rounded corners bearing the right-facing, diademed and draped effigy of Queen Elizabeth II after the portrait by Ian Rank-Broadley, whose engraver's initials 'IRB' appear below the truncation. The legend 'COOK ISLANDS' and denomination '5 DOLLARS' are inscribed in two lines at the top of the field, while the name 'ELIZABETH II' is engraved in bold capital letters along the lower margin. |

| Reverse description | Full-color reproduction of the Russian Orthodox icon of the Mother of God known as the 'Inexhaustible Chalice' (Неупиваемая Чаша), depicted in the Orans posture with both arms raised and a large gilded eucharistic chalice before her, within which the Christ Child is shown in benediction. The Virgin is shown in a deep crimson maphorion with blue highlights, crowned by an ornate golden crown, and surrounded by an elaborate filigree-style silver border framing the entire composition. A Cyrillic inscription identifying the icon is engraved in a decorative cartouche at the bottom of the inner border, with the date '2013' appearing in the lower margin outside the decorative frame. |
