Obverse description Right-facing diademed and draped effigy of Queen Elizabeth II, engraved by Ian Rank-Broadley, set against a mirror-polished proof field shaped to the outline of Pope Benedict XVI's bust in left profile. The legend COOK ISLANDS arcs above the portrait along the upper rim, ELIZABETH II is inscribed vertically along the left edge, and the denomination 5 DOLLARS appears in the lower field below the effigy.

Reverse description Left-facing bust of Pope Benedict XVI in high relief, depicted wearing a papal zucchetto and ornate gold-gilded vestments. Seven Swarovski crystals — five red and two blue — are affixed diagonally across the pontifical cope, adding chromatic contrast to the gold-gilded drapery. The shaped flan follows the contour of the papal bust precisely. A rectangular exergual panel at the base bears the Latin inscription commemorating the Pope's 80th birthday, arranged in two lines.
