Obverse description Third-portrait crowned effigy of Queen Elizabeth II facing right, modelled by Raphael David Maklouf, depicting the Queen wearing the George IV State Diadem and drop earrings, with a necklace visible at the truncation. The legend ELIZABETH II is inscribed along the left periphery and COOK ISLANDS along the right periphery, both in raised Latin characters. The date appears in the lower exergual area below the portrait. The engraver's initials RDM are incuse on the truncation.

Reverse description A highly detailed, naturalistically rendered conch shell (Charonia tritonis or similar Pacific species) depicted in high relief, oriented diagonally across the field with its spired apex pointing toward the upper left and the lip of the shell extending toward the lower right. The denomination $ 5 is inscribed in the upper right field. The engraver's monogram HH appears incuse at the lower centre of the field below the shell.
