Obverse description Right-facing effigy of Queen Elizabeth II wearing a diamond tiara and pearl drop earrings, with a draped neckline. The portrait is rendered in high relief against a deeply mirrored proof field, consistent with the Raphael Maklouf third portrait used on Commonwealth coinage of this period. The legend ELIZABETH II arcs above and COOK ISLANDS curves along the right periphery in raised Latin lettering.

Reverse description Central large numeral 4 flanked by the date split as 20 to the left and 02 to the right, all enclosed within a wreath of laurel branches tied at the base. A royal crown surmounts the composition at the top of the wreath. The design closely follows the traditional English Maundy coinage reverse style, adapted here for the Cook Islands issue. The entire device is set against a highly polished proof field with a milled border.
