Obverse description Vertically oriented note printed in blue with red accents on cream paper. The border is composed of intricate foliate and geometric guilloche ornaments with dragon-motif corner pieces, enclosing a pale pink underprint of interlocking square key-fret patterns. At the top, the issuer's name is inscribed in black Chinese characters above a cartouche bearing the denomination '三泉湧' in bold red script; the main body carries the payment obligation text in large black Chinese characters, with the date fields (Republic year, month, day) and serial number fields left blank for hand-completion. A red seal impression reading '三泉湧記票' appears at lower left, and the printer's imprint is letterpress-rendered along the bottom margin.

Reverse description Vertically oriented reverse printed entirely in blue on cream paper, enclosed by a multi-rule border with geometric guilloche bands and circular medallions at each corner bearing the anti-counterfeiting characters '謹', '假', '票', '防'. The upper register contains a large oval floral guilloche medallion enclosing the numeral '叁' (three), flanked by two pointed cartouches reading '概不掛號' and '失票不管' respectively, beneath the header inscription '三泉湧'. The lower register is occupied by a detailed landscape vignette in the Chinese literati tradition, rendered in fine line engraving and showing cascading waterfalls, rocky outcroppings, and trees.
