Obverse description The obverse is set within a double-rule border with red and black vertical striped panels at left and right, evoking the Imperial German flag. At upper centre, a bold typeset heading reads 'GUTSCHEIN ÜBER ZWEIHUNDERT PFENNIG' within a rectangular panel, below which a serial number panel appears. Two military medals on ribbons flank the central text field: on the left, a round medallion with a figurative scene, and on the right, a round medallion bearing an Iron Cross motif. The issuing authority 'DER ORDENSRAT IM VERBAND NATIONALGESINNTER SOLDATEN' is printed centrally alongside a manuscript facsimile signature and the title 'KGL. HAUPTMANN A.D. ALS SCHRIFTFÜHRER'. At the lower margin, a caption reads 'Deutsche Ehrendenkmünze des Weltkrieges mit dem Kampfabzeichen', with the printer's imprint 'M. PATHE' below.

Reverse description The reverse is bordered with Iron Cross vignettes and oak-leaf underprint panels at the lateral margins, each bearing the denomination '200 PF.' within heraldic shield cartouches in black on white. The central vignette presents a detailed pen-and-ink style architectural view of the Jakobikirche in Thorn (Toruń), accompanied by a circular medal showing an Iron Cross with the legend 'FÜRS VATERLAND'. A curved banner at the top carries the inscription 'An der Weichsel gegen Osten'. Below the architectural vignette, a label reads 'Thorn. Jakobikirche.' A three-line redemption text in small letterpress type at the foot of the note states the conditions of validity, referencing the trustee C. Brückmann, Berlin, Hollmannstraße 18, and the expiry date of 20 April 1922.
