| Obverse description | The obverse features a stylized rectangular panel rendered in relief to evoke the texture of wooden planking, occupying the central field and referencing the timber construction of the Haczów church. Superimposed upon this panel is the crowned Polish White Eagle, the national emblem, with wings displayed and talons extended, rendered in finely detailed relief.
