Obverse description At center, a circular vignette bears a silhouette portrait of Henry Massie, founder of Portsmouth, Ohio, set within a decorative mural composition. The numeral '5' appears flanked on either side of the central vignette. Surrounding text inscriptions record the 150th Anniversary of the Northwest Territory and the Portsmouth, Ohio celebration dates of October 2–6, 1938.

Reverse description The reverse is divided into three vertical panels: the left panel carries a scenic vignette of an early locomotive and rural landscape, the right panel a vignette of a riverside town with buildings and a boat, and the central panel contains an oval text block with the redemption terms. The numeral '5' appears at upper left and upper right within decorative scroll cartouches. Two facsimile signatures appear below the central oval, with the serial number printed in red beneath them, and the bold legend 'ONE WOODEN NICKEL' runs across the lower portion.
