Beschrijving voorzijde The obverse features the fourth definitive effigy of Queen Elizabeth II facing right, as modelled by Ian Rank-Broadley, wearing the Girls of Great Britain and Ireland tiara and a drop earring, with a draped neckline. The legend ELIZABETH II is inscribed to the left and COOK ISLANDS to the right, with the date 2003 at the bottom of the field. The engraver's initials IRB appear at the truncation of the bust. A continuous inner border of raised beads encircles the design.

Beschrijving keerzijde The reverse presents a colourful polychrome depiction of the Cancer zodiac sign, rendered as a kneeling female figure dressed in ornate traditional costume and a pointed hat, holding aloft a large crab. The Cancer astrological symbol and the legend 'Cancer' appear in the upper left field, while two small red crabs are shown at lower left. The denomination 50¢ is inscribed in the lower left area of the field. The central design is set against a black background within a silvery stippled border ring.
