Obverse description The inner disc depicts two facing figures in medical attire — a male and a female healthcare professional — each wearing surgical masks and shown with a stethoscope and a medical folder, symbolising doctors and nurses engaged on the front line during the COVID-19 pandemic. To the upper left of the composition appears a medical cross symbol, while to the upper right is a heart symbol, with the word GRAZIE (meaning 'Thank You') arching above between them. In the upper centre of the field, the bipartite inscription RI identifies the Italian Republic, and the mint mark R for the Rome Mint appears to its right. The designer's initials C.M. for Claudia Momoni are inscribed to the lower left, with the date 2021 placed in the exergue.

Reverse description The standard European Union reverse, designed by Luc Luycx, features a relief map of Western Europe spanning both the inner disc and the outer ring on the right side of the coin. The denomination 2 EURO is inscribed across the design, with the numeral '2' positioned in the open field representing the eastern Atlantic Ocean. Twelve five-pointed stars, symbolising the member states of the European Union, are arranged along the right portion of the outer ring — six above and six below the continental map. Six vertical relief lines traverse the inner core, visually linking the upper and lower star groupings. The engraver's initials LL appear to the right of the denomination.
